Airborne dust is the factor specific to noses

Particulate air makes that worse. It provokes sneezing and the urge to blow, both of which are precisely what you were told to avoid in the early weeks.
Northern Thailand has a well-documented burning season, roughly February to April, when agricultural burning raises particulate levels considerably. That does not make Chiang Mai the wrong choice. It makes the season a real part of the decision in a way it would not be for a facelift or a tummy tuck.
How long you actually stay
This decides more about the trip than the city, and it is set by the splint rather than by the address.
An external splint usually stays on for about a week, and its removal is a clinic appointment that also checks the wound and confirms there is no bleeding.

The first night, and where to stay
If your operation is at the clinic, the sensible arrangement is to be a short journey away that evening rather than across a city in traffic.
- A short, predictable journey to the clinic for the splint review.
- Air conditioning you can turn down, since very dry air irritates a healing nose.
- Windows that stay closed, on days when the air outside is poor.
- Enough pillows to sleep propped upright for the first week.
- A fridge or reliable ice, for cool compresses around the eyes.
- Food available without a long walk in dust and daylight.

Timing the trip across the year
Season matters differently here than it would for a holiday, because you will spend the first week indoors either way.
November to January is the most comfortable window in both cities, and the northern air is at its clearest.
February to April is hot everywhere and is the northern burning season, which is the one period where the two cities genuinely diverge for this operation.
May to October brings rain and humidity, which affects getting to your appointment more than it affects healing, and keeps particulates down as a side effect.
The journey home from each
A connecting flight matters less after rhinoplasty than after abdominal surgery, and it is worth saying so plainly.
What a domestic leg adds is more hours in very dry cabin air, one more set of pressure changes on descent, and a transfer through a crowded terminal while your face is still healing.
None of that is dangerous. It is a reason to carry saline in hand luggage on both legs and to protect your face in boarding queues.
Give your surgeon the real itinerary rather than a total number of hours, since two legs and a layover is a different journey from one direct flight.
